If an attendee has a disability that requires special services, we are prepared to accommodate him/her. However, unless advance notification is received, we may not be able to provide such services.
FOR INTERNATIONAL ATTENDEES: Letters of invitation for presentation to embassy officials, to apply for travel visas, will be made available upon request. We have registered this meeting with the U.S. Department of State and encourage foreign participants to apply for their visas as early as possible - at least 3 months before the meeting (by September 11, 2009). Some consulates may have backlogs in scheduling visa interviews so applicants should first contact the consulate to find out how long the wait is for an interview. Visitors from participating Visa Waiver Program (VWP) countries should be aware of Machine-Readable Passport (MRP) requirements. Since New York City is a major entry port for international arrivals, you may experience some delays at passport checkpoints. We appreciate your patience and understanding.
CLIMATE FACTS and DRESS CODE
The normal average outdoor temperature in December for New York City fluctuates from 31°F (-01°C) to 42°F (6°C). Due to indoor climate control variables, for your comfort, you may wish to bring a sweater or light jacket when attending sessions. The customary dress code for PGA attendees is smart casual - unless unusual weather circumstances dictate otherwise. PGA Speakers are required to wear business attire.

Payment of the general registration fee includes admission and access to:

Additional admission fees are required for optional sessions such as Interactive/Hands-On Workshops, Miniworkshops and Problem-Based Learning Discussions. Since these sessions are conducted in facilities that have limited seating, it is recommended that you pre-register your selection(s) in advance of the meeting.
- Only duly registered individuals with appropriate badges may attend convention events.
- No Fee/Non-Medical Guests of Registered Physicians and No Fee/Non-Medical Guests of CRNAs will not be permitted access to scientific sessions.
- There are no provisions for one-day registration fees.
ADVANCE REGISTRATIONS MUST BE RECEIVED BY DECEMBER 1st.
After December 1st, we will no longer be able to accept or process advance registrations. Thereafter, registration will commence on-site at the New York Marriott Marquis on Thursday, December 10th at 4:00 pm (1600) on the Fourth Floor.

NYSSA MEMBER REGISTRATION
Information You Need to Know
All NYSSA Members (Active, Affiliate, Resident, Retired, Honorary & Medical Student) should go directly to the NYSSA Members Only Advanced & On-Site Registration Stations, located on the 4th floor promenade of the Marriott Marquis, to claim their personalized meeting materials. NYSSA members who registered in advance, please present the acknowledgement form that was previously mailed to you. If you do not complete and return advance registration forms before the deadline and you choose to register on-site, your membership will have to be verified upon presentation of your current membership card. Any discrepancies will require review. To avoid delays, it is suggested that you register in advance.
APPLYING FOR NYSSA MEMBERSHIP
In order to be eligible to register as a pre-paid NYSSA member for this PGA you must be a physician anesthesiologist, resident or medical student in practice or in school in the State of New York. Qualified individuals wishing to join the NYSSA must file a membership application together with the required documentation. These forms must be received at NYSSA Headquarters Office on or before November 1st, and will be subject to final processing/approval. Applicants who file after November 1st, will not be eligible to register for this PGA in the NYSSA member category, and the appropriate non-NYSSA member PGA registration fees will be in effect.
The New York Marriott Marquis (1535 Broadway, between 45th & 46th Streets, New York City) is the official Headquarters Hotel and serves as the site for all PGA meeting activities. Arrangements for special convention rates have been contracted with The Marriott and with several other midtown hotels.
All requests for hotel room reservations must be mailed directly to BORRELLI TRAVEL SERVICE, INC. 2820 Dewey Avenue, Rochester, New York 14616, faxed to 1-585-663-5155 or submitted online. If you need additional information and you are calling from the U.S. and Canada dial 1-800-842-0941 and if calling from International locations dial 1-585-663-8660 - please refer to PGA. Sending Hotel Reservations to NYSSA Headquarters will delay its processing.
December is an extremely busy time in New York City. We suggest that you complete and return your Housing Form as quickly as possible. Every effort will be made to secure the hotel accommodations of your choice.
Changes and cancellations in your reservations will be handled by Borrelli Travel
. Thereafter, you should notify your assigned hotel of any changes in your itinerary. You may be subject to penalty charges.
The major portion of our reserved room block is with the New York Marriott Marquis. However, we do offer limited alternative housing at several other hotels in the vicinity of the Marriott. In the event accommodations are not available at the hotel(s) of your choice, we reserve the right to instruct the Housing Bureau (Borrelli Travel) to assign you to the next available hotel that best meets your needs.
Approximately four weeks prior to the opening date of the PGA, special convention room rates will no longer be applicable and accommodations can then be secured only on a space/rate/availability basis.
FACULTY DISCLOSURE
The PGA insists that balance, independence and objectivity be given at all didactic sessions. Therefore, all PGA speaking faculty as well as Program Committee organizers have been asked to disclose any potential conflict of interest with respect to their presentation or their program content responsibilities. This will be noted in the final program-journal distributed at the meeting, as well as on display in meeting rooms with A-V projection -&-verbally stated at the beginning and end of all general and optional sessions. The views and opinions expressed by speakers or others who have provided materials to and for this meeting may not necessarily represent the views, opinions, policies or actions of the PostGraduate Assembly in Anesthesiology or The New York State Society of Anesthesiologists, Inc. The PGA/NYSSA assumes no responsibility for, nor endorses any of the comments, recommendations or materials that are provided.

Lunch Cart Service will be located within the Technical Exhibit complex located on the 5th Floor each exhibiting day from 11:30 am until 1:30 pm (1130 - 1330). The only exception is on Monday when this service will conclude at 1:00 pm.

AIRPORT TO HOTEL GROUND TRANSPORTATION
New York Airport Service provides motor-coach transportation to and from LaGuardia and JFK airports as well as to and from most major mid-town hotels in New York City. The current standard one-way fare in effect as of March, 2007, was:

- JFK to New York City - $15 per person

Olympia Trails provides service to and from Newark Airport. The current one-way fare to various drop-off and embarkation points in New York City ranges from $10 to $15 per person. Call 1-212-964-6233 for additional information.
Taxi fares from the three metropolitan airports to mid-town New York City range from $45 to $55 one-way.
SOCIAL EVENTS and SPOUSE PROGRAMS
Included in this PGA is a diverse entertainment and tours section planned by the PGA Local Arrangements Committee.
PLEASE NOTE: Pre-registration for these activities must be submitted together with the PGA Advance Registration Form, otherwise this will delay final processing. Since tickets are limited, failure to submit proper forms could result in forfeiture of your selections.
Information regarding Broadway theatre plays, shows, museums and restaurants may be obtained at the PGA Social Activity Tour Desk located on the 4th floor of the Marriott. Keynote personnel will be on duty during official registration hours to assist you. Please come by and pick up any of the free information pamphlets.
